Early on in implant days we learned that titanium would integrate with bone. Now, we're reaching incredible levels of bioengineering in threads, implant surface and attachments. An example is this DS prime taper implant from Dentsply Sirona Notice the difference in thread design from apex to crown. Different threads engage the bone differently and are designed to respect the interface with the gingiva at the coronal end, with a change in thread design towards the apex for stabilizing, immediate load and ease of placement. Implant biomaterials science is an exciting frontier.

Scans have different challenges than impressions. The great news is that your scan software will often highlight areas that need a little touch-up, and it's easy to add on without starting over.

Design Only By far, the most popular option for our dentists is design and print. You send the CBCT scan and Intraoral Scan (or models) and we create a proposal for your approval. Once you approve the proposal, we generate the guide and you're ready for surgery. Some dentists have their own DTX software and like to take the first step in their office. These clinics do their own plan and design and share the design with us to generate the guide. Although less common, some clinics send us the CBCT and Intraoral Scan for the initial proposal and have us send an *.STL instead of a completed guide. "Digilog" is the best of the DIGItal and the anaLOG process of records and manufacture. Dr. Scott Kirby in Fredericton is an absolute master of the digilog denture process. Digital records allow for each step to seamlessly build upon the last and have a memory. It allows for trial prosthesis that accurately mimic how the true prosthesis will look and fit. Analog records allow for tissue contact and adaptation. They allow the dentist to control the contact pressures on soft tissues and borders. Using an existing denture or a try-in with a wash impression the clinician can capture an excellent border-molded record. In this case, Dr. Kirby used a printed try-in as a "very close" prosthesis. This serves as a record of vertical dimension, midline, tooth position and as a custom tray to capture a border-mold and final impression (is that a little bit of greenstick compound we see in the buccal shelf?) This analog record is stitched with the existing digital record of the tooth set-up. Final process fuses the new analog record with the existing tooth set-up for a totally seamless integration with the existing digital workflow. As an added bonus, when the wash impression is scanned chairside it can be electronically sent from Fredericton to our lab in Halifax in less than a minute (It's hard to find a courier to do that!).

This type of case is life-changing. The surgeon chose to use five Straumann implants with SRAs. These SRAs draw from multiple angles allowing multiple units to seat without binding.

A study wax-up is a proposal for treatment. This case shows what it would look like if the patient opted for this treatment. Sometimes patients like to see and hold their models as they consider treatment. Sometimes the dentist uses the printed models to visualize treatment from multiple angles and perspectives. Printed proposal models are an excellent tool for communication between the dentist, patient and lab technician. Expectations can be managed before the case is even started.
